People with Disability Survey: The Voice of Queenslanders with Disability 2025

It will take about 30 minutes to complete this survey. Share as much or as little as you want. To complete the survey using this document, you will need to fill out the questions. Please complete as many questions as possible. Any question with an asterisk (*) means that it needs to be answered. There are lots of ways you can share your story. If you don’t see an option that works for you below, please let us know. Download the survey and choose how you would like to complete it:

· Print a paper copy, fill it out and mail or email it back to us.

· Fill out the survey on your device and mail or email it back to us.

· Speak with us or sign over the phone, in person, or via video call (Zoom, Teams or FaceTime). We can provide interpreters.

· Request a paper survey to be mailed to you, which you can complete and return by post.

· Answer the questions in an audio or video response and email it to us.
