Submit audio, video, or text responses via email.
Speak with us or sign to us on the phone, in person, or via Zoom or Teams. Interpreters are available.
Request a paper survey to be mailed to you, which you can complete and return by post.
Contact us if you need the survey in an alternative format.
Who is running the survey: Griffith University researchers in partnership with Queenslanders with Disability Network (QDN) and the Queensland Government.
Your privacy matters. Your information stays confidential and is only used to follow up if needed.

It will take about 30 minutes to complete this survey. Share as much or as little as you want. To complete the survey using this document, you will need to fill out the questions. Please complete as many questions as possible. Any question with an asterisk (*) means that it needs to be answered. There are lots of ways you can share your story. If you don’t see an option that works for you below, please let us know. Download the survey and choose how you would like to complete it:
