Family and Carers Survey 2026

About this survey

This survey has three main parts. Because the survey is about you, the number of questions may change as you go through it. There are 45 questions in the main part of the survey at a maximum.

• There are 30-31 questions about your life. These are quick questions where you click a button to tell us if you agree or disagree.
• There are 1-15 optional questions where you can type more about your stories into the space provided. You only see the questions for the topics you want to share more about.
• There are 10-12 questions about you. These are short questions about things like your age and where you live.

You can share as much or as little as you want. Any question with a star (*) must be answered to move on.

Saving your progress

Because this is a long survey, you might want to take a break and finish it later. To do this, you must Sign Up before you start the survey.

2. Enter your email, choose a password, and answer one question

3. Check your email for an activation link. You must click this link to finish your sign-up.

4. Log in to the website and then start the survey.

Note: If you leave and come back later, the survey will open at the first page. Don't worry, your answers are still there! Just click "next" until you reach the page where you stopped.
