Who is running the survey: Griffith University Researchers in partnership with Queenslanders with Disability Network (QDN) and the Queensland Government.
Your privacy matters. Your information stays confidential and is only used to follow up if needed.
Before you start, please read the Participant Information Form. It explains the study, who can join, how your information is kept private, and your rights. Download or save it for later.
It will take about 30 minutes to complete this survey. Share as much or as little as you want. You cannot save your answers as you go, but you can take breaks and come back to the web page. If you would like to do the survey in smaller parts, please email dignityproject@griffith.edu.au.
