When is the survey open?

    Who can participate?

What types of questions will be in the survey?

This survey has four types of questions:

    1. information about you (age, gender, disability type, etc.)

    2. a chance for you to let us know what is happening in different areas of your life

    3. a way for you to tell us if these areas are getting better, worse or staying the same

    4. space for you to provide more information about different areas (for example, what is going well, what is not going well and what would improve your life in this area).

You can share as much or as little as you want.

Will anyone be able to see my information or link my answers to me?

Personal information collected as part of this survey is collected in the case that we need to contact participants to follow up on their responses. Any information participants provide will be kept confidential and will not be passed onto a third party.
